Student Assistance Services- The Health And Wellness Case Manager

The Health and Wellness Case Manager is a licensed social worker who is here to assist students with a variety of concerns that impact daily functioning. The Health and Wellness Case Manager is different from a therapist or counselor. A case manager assists students with making connections to appropriate resources and services while counselors provide therapy to help students gain better coping skills and understanding of their experiences, symptoms, and behaviors.

Possible areas of case management support include, but are not necessarily limited to:

  • working with students to understand options and to make informed decisions.
  • assisting students gain information and provide guidance to navigate community and/or University resources.
  • providing support with communication, outreach, and collaboration with community and/or University resources.

Making an Appointment

Appointments can be made by phone, in person, or by email any point during the year. Appointments are available Monday-Friday during business hours. It is asked that initial appointments take place at the Health and Wellness Center, but after that the Health and Wellness Case Manager can meet at any public place on campus. Every attempt will be made to schedule an appointment as soon as possible.

Sometimes, The Health and Wellness Case Manager will receive a referral for students from a concerned professor, friend, or other ESU staff/faculty and will reach out directly to the student to offer resources, support, and an appointment.

The Health and Wellness Case Manager is available during drop in hours at the Health and Wellness Center. Drop In time can be utilized for setting up an appointment, meeting with the Health and Wellness Case Manager, or just asking questions and saying “Hello!”

The Case Manager will also be available to students during CAPS Walk-In hours to assist students with case management needs. Remember, the Case Manager is available to help any student with acquiring resources, making informed decisions, and making meaningful connections on and off campus!

What Types of Activities May Take Place During an Appointment

Each session will be individually tailored based on the purpose of the visit and the needs of the student, but there is some general information that The Health and Wellness Case Manager will need in order to best serve. This information includes the student’s name, ID number, and contact information. This information is collected so that The Health and Wellness Case Manager has the most up to date information as to where to contact the student for follow up, emergency contact, and to maintain records.

What does a case manager do?
The Health and Wellness Case Manager assists students with making informed choices, develop and reach their goals, and navigating University and/or community resources, just to name a few!
When is the case manager available to meet?
The case manager has scheduled office hours, drop in hours, and available by appointment.
Why was I referred to The Health and Wellness Case Manager?
Someone who cares about a student will ask The Health and Wellness Case Manager to reach out directly to the student to offer resources, support, and/or an appointment. It is all part of an effort of all the ESU faculty and staff to help students find success, maintain safety, and have the best experience possible.
Is it a confidential service?
The service is confidential, but there are times where the Case Manager may have to break confidentiality, for example if a student is expressing threat to harm themselves or others or a child is being abused.
What is the difference between a case manager and a counselor?
The Health and Wellness Case Manager supports students in gaining access to resources and connections to supports outside of themselves, while a therapist or counselor provides advice, support, and utilizes therapeutic modalities to increase insight, coping skills, and help the student improve themselves on the inside.
I am transitioning to college from foster care. Can Case Management help me?
Yes! Please contact the Case Manager as soon as possible via phone, email, or drop by the office to discuss what resources, programs, financial aid, etc. is available to help you be successful at ESU.
