This form is for individuals that are interested in being a CILLS Mentor. The Career, Independent Living and Learning Studies Program is a 3 year certificate program for individuals with disabilities hosted at ESU.

Weekday Mentors: Hours could be Monday-Friday 4:00 p.m. - 9:00 p.m. (Based on your schedule!) Responsibilities include, but are not limited to; helping students with their academics, taking the students to social events (sports/clubs), making sure the students are doing their daily living responsibilities, safety of the students, communication with staff, etc.

Weekend Mentors: Hours could be Saturday-Sunday 11:00 a.m. - 4:00 p.m. or 4:00 p.m. - 9:00 p.m. (Based on your schedule!) Responsibilities include, but are not limited to; doing activities that the students want to do on the weekends, helping the students with academics if needed, assisting the students cook lunch/dinner, assisting the students with daily living skills, safety of the students, communication with staff, etc.

You are allowed to be both a weekday and weekend mentor.
