The purpose of the Condom-Gram is to increase the amount of students who consistently engage in safer sexual behaviors by the use of barrier methods, increase partner consensual communication, and increase the number of students to be tested for sexually transmitted infections and diseases.

By participating in the Condom-Gram, please understand that the information provided on the survey is solely used for evaluation purposes for the Wellness & Education Prevention office.

The procedures to follow include: filling out a pre-survey, receiving/using the Condom-Gram, and filling out the post survey.

Please be aware that this program could make participants feel uncomfortable when answering questions regarding your own sexual health.

Confidentiality will be maintained by having password protected files, only allowing the Wellness & Education Prevention center to have access.

When Condom-Gram packages are in transportation to the participant from the Wellness & Education Prevention Office to the mailing center on campus, all packages will be inside a non-transparent Rubbermaid container to also maintain confidentiality.

Please note, confidentiality could be at risk if something is to go wrong during the procedure. If you wish to stop the survey at anytime, you may do so.

Participation for the Condom-Gram is voluntary and participants can withdraw consent at anytime without penalty.
