University Immunization Policy

All matriculated students are required to provide evidence that they have been appropriately immunized upon admission to the university.

Official documentation must be provided on the Report of Medical History form and submitted to the Health and Wellness Department for measles (rubeola), mumps, rubella (German measles), tetanus /diphtheria or tetanus /diphtheria /acellular pertussis, meningococcal disease, polio, tuberculin testing, and hepatitis B series.

Laboratory evidence of immunity is accepted for measles, mumps, rubella, and hepatitis B.

Pennsylvania law requires all students residing in university-owned housing to provide proof of meningitis vaccination or have a waiver of exemption for medical, religious, or personal belief reasons.

Failure to submit the completed Report of Medical History form will result in a hold being placed on the student’s university account which will prevent university transactions until the completed form is submitted.

Students who have not had one or all of the required immunizations due to medical, religious, or personal beliefs must complete an affidavit or waiver approved by University Health Services to be granted exemption and may be asked to leave the university during an outbreak of one of more of the diseases.

Reason for Policy

Vaccine preventable diseases continue to occur in America’s college campuses.

The PA Department of Health, the American College Health Association, and the Advisory Committee on Immunization Practices recommend mandatory immunization requirements for all matriculated students.

In response to changing epidemiology and the introduction of new vaccines, these requirements are updated accordingly.
